Session Description

You just made an extraordinary commitment to your health and wellness. Over the next 4 weeks I will provide you with suggestion, on how to fuel your body with good whole foods. Based on your individual goals/intentions. Keeping up the hydration. Along with eliminating things that aren’t serving you or your goal. Share successes and overcome defeats. It’s time to “Add health to your years.” Please read and sign the following agreement: Program acknowledgment: This Agreement is made between Tara Ivins and the person named at the end of this document, [the Client]. The Nutrition Coaching Program in which you are about to enroll will include all the following: A. Two 30 sessions held over the phone or via Zoom biweekly for 4 week period B. Weekly Check-ins via text COACHING SESSIONS SCHEDULING: Please be on time. The session starts at the designated time. I will wait 10 mins if you have not arrived within that time. The session will be considered fulfilled- no rescheduling. *If the Client needs to cancel or reschedule the appointment, the Client must do so 24 hours in advance; otherwise, the Client will forfeit that appointment and will not have an opportunity to reschedule it. This program expires if the 2 sessions have not been completed within 1 week after the End Date specified. PAYMENTS AND REFUNDS The Client understands that the cost of the Program is $130. every 4 weeks that is for two 30-minute phone or Zoom sessions and weekly check-ins via text. In the event of the Client’s absence or withdrawal, for any reason whatsoever No refund for any remaining sessions unfulfilled. The Coach reserves the right to cancel the program if at any point she or he feels it is not advantageous for the coaching program to continue. I will return money for any remaining sessions not used. Tara Ivins Statement of Intent: My first appointment with you will be multi-faceted, going over the structure of the program, and look over any documentation you have provided. I agree to do the following: • Provide a food & lifestyle form and assess your unique needs. • Discuss your specific goals. • Make specific recommendations as necessary. Client Acceptance of Responsibility: I have been informed and understand the nutritional and lifestyle recommendations. In addition, I agree to do the following: • Submit Food and Lifestyle form including activities (daily) • Keep coach up to date on life happenings and activities • Utilize the food recommendations
